Kynett's wife Edna, filled the form in Kynett's behalf and has ticked the second and third options. In responce to the final three checkboxes, Edna has written that Kynett prefers the middle nurse, who works "between day and night", and that Kynett will be in hospital for another month at least. Edna has also filled in a section for "Remarks", which McBey created, to update him further on Kynett's condition.
The survey consists of one sheet handwritten in pen and pencil on both sides.
